The parent facilitator completes the competency assessment after reviewing and discussing progress with the parent. Areas of strength will be noted, and areas that need attention or review will be noted. A plan for reinforcement of concepts will be discussed and implemented. This form will be filled out to record progress of the parent in meeting competency requirements.
